In many parts of the world, the HIV epidemic falls particularly hard on women. This is certainly the case in sub-Saharan Africa, where in 2012 fifty-nine percent of all those living with HIV were women, in a region where fifty-three percent of the general population is female.
Gender inequality is a major contributor to the prevalence of HIV infection among women: women and girls often face limited access to education, employment, and healthcare. Men often dominate sexual relationships. Gender-based violence has been identified as a key driver of HIV transmission.
Marriage often provides no safe haven. Studies in Kenya and Zambia have shown that marriage increases the frequency of sexual intercourse for women, while hindering their ability to negotiate safe sex or to abstain. It is also common in many parts of the world for men to have multiple sexual partners even when married. The lack of control faced by women in these relationships puts them at high risk of infection and unwanted pregnancy.
In addition to increased education and improved legal protection, women could benefit from a method of delivering contraception and HIV protection that they could control.
